2) The primary purpose of documenting care in client records is for communication between healthcare providers so that they can plan appropriate care. However, there are additional uses of the information found in the client’s record. Discuss how complete, accurate, and clear documentation can be used in auditing for quality assurance or continuous quality management, reimbursement, and healthcare analysis. Be sure to cite examples to support your observations
3) Recall an experience in which the change process was difficult and disruptive. Explain why this change was difficult to accept. What advice would you give others about how not to get stuck in resistance and nonacceptance of change? What lessons have you learned about change?
